Description of osteochondritis dissecans
The condition is defined as a rejection of the bone and cartilage in the knee joint and is often called “mouse in the knee”. It is most commonly located on the outside of the inner thigh bone’s head / condyle (femur condyle). The cause of the disease is rarely known. Sometimes direct trauma and distortions in the knee may cause an osteochondral fracture / fracture that does not heal – but develops into an osteochondral. The trauma can gradually evolve so that the altered area or portions thereof loosen from the bone and give rise to lose fractions of bone in the joints (“mice”).
Symptoms of osteochondritis dissecans
- Pain connected with severe stress, especially at hyperextension (extreme knee joint extension).
- Effusion / swelling that is indicative of a light condition (synovitis).
- Lock-up of the knee.
- Diffuse pain in the knee.
- Inspection of the joints through arthroscopy may be indicative of the treatment plan. Possibly. “Mice” can be removed via arthroscopy.
Examination of osteochondritis dissecans
- Analysis / inspection / palpation / movement test.
- Wilson’s test (twist the lower leg under stretch and bend of the knee joint).
Treatment of osteochondritis dissecans
The treatment is usually conservative, i.e. Relief from knee loading sports, often for months.
The doctor may often have to immobilize with cast for approx. 6-8 weeks, and hope for natural healing of the osteochondral remnant (mouse). If healing has not occurred, the forecast is generally poor. In some cases, it may be necessary to remove the bone fragment and thus leave a defect in the joint. The surgery can often be done trans-arthroscopically.
